Two signals, one goal: get chronically painful tissue pain-free again.

Chronic tendon and fascia pain often isn't ongoing damage - it's tissue that has healed poorly, frequently resulting in scar tissue that is strong but still symptomatic. Both treatments are designed to give it a fresh stimulus to repair.

Extracorporeal shockwave therapy · ESWT
Shockwave

Short, high-pressure acoustic pulses travel through the skin into the injured tendon, fascia, or bone. Cells respond to that mechanical stimulus with new blood-vessel growth, collagen remodeling, and a reset of local pain signaling - kick-starting repair in tissue that had plateaued. It's one of the most-studied non-surgical options for chronic tendon pain.

  • Feels likeA strong, rapid tapping over the sore spot. Intensity is set to your tolerance.
  • TargetsThe precise spot that hurts - we locate it by feel and feedback.
  • EvidenceDecades of research, including multiple randomized trials.
⬡
Extracorporeal magnetotransduction therapy
EMTT

High-energy pulsed electromagnetic fields - far stronger and faster-oscillating than conventional PEMF devices - reach deep into muscle, tendon, and bone without any contact pressure. The field is thought to stimulate cell metabolism and repair across a broader area than the shockwave head covers.

  • Feels likeLittle or nothing - at most a light twitch or tingle. It's painless.
  • TargetsA wider, deeper zone around the injury, including bone.
  • EvidenceNewer. Early randomized trials are promising, but the research base is much smaller.
+

Why pair them? Shockwave treats the focal point of the injury; EMTT treats the tissue around and beneath it. Combining the two in one visit is a common protocol - and combination trials are where EMTT has shown the most benefit so far, e.g. better outcomes than shockwave alone in a randomized trial for rotator cuff tendinopathy.

Common running injuries, rated honestly.

Shockwave isn't a cure-all. Here's where the research is strong, where it's thin, and where it's mixed.

Injury
Evidence
What the research says
Plantar fasciitisHeel and arch pain
Well supported
Multiple randomized trials and meta-analyses support shockwave for chronic heel pain that hasn't settled with standard care.
Achilles tendinopathyMid-portion and insertional
Supported
Works best alongside a loading program - eccentric loading plus shockwave beat loading alone in a randomized trial.
Lateral hip painGreater trochanteric pain syndrome
Supported
Randomized trial evidence for shockwave over usual care, with longer-lasting results than a corticosteroid injection.
Proximal hamstring tendinopathyDeep buttock pain
Early
One randomized trial in professional athletes was clearly positive; it hasn't been widely replicated yet.
Shin splintsMedial tibial stress syndrome
Early
Non-randomized studies show faster recovery when shockwave is added to a graded return-to-run plan.
Patellar tendinopathy"Jumper's knee"
Mixed
Some trials show gains in chronic cases; one trial in athletes treated mid-season found no benefit over placebo.
Bone stress injuriesStress reactions, slow-healing fractures
Early
Small studies only. Needs a medical diagnosis - and your physician's OK - before treatment.

Ratings reflect the shockwave research. EMTT's evidence is earlier-stage across the board.

The training plan keeps it going.

Tendons get irritated when load outpaces adaptation. The best results in the research come from pairing treatment with progressive loading - which is why our coaching athletes get both.

Keep training, smarter

Mild soreness for a day or two is normal. Most runners keep training as symptoms allow - easy days first.

Most protocols run 3–6 sessions, roughly a week apart. We reassess your symptoms each visit rather than locking you into a package.

Safety first.

Shockwave and EMTT are low-risk for most healthy runners, but they aren't for everyone.

Good candidates

You'll likely get the most out of it if:

  • The pain has lasted roughly three months or more
  • It's a tendon, fascia, or known overuse injury - not a mystery
  • Rest, rehab exercises, or load changes alone haven't resolved it
  • You're willing to pair treatment with smart training changes
Don't book if any of these apply

Talk to us first - and in most cases your doctor:

  • Pregnancy
  • Pacemaker, defibrillator, or other implanted electronics
  • Metal implants in or near the treatment area
  • Bleeding disorder or blood-thinning medication
  • Cancer, or a tumor near the treatment area
  • Infection, open wound, or skin condition at the site
  • Steroid injection in the area in the last 6 weeks
  • Under 18 (open growth plates)
  • A new, undiagnosed injury - e.g. a suspected fracture or tear

Good questions.

Does it hurt?

Shockwave is uncomfortable but brief - a strong, rapid tapping right over the sore spot. We start low and adjust to your tolerance. EMTT is painless; most people feel little or nothing.

How many sessions will I need?

Most protocols are 3–6 sessions, about a week apart. Some runners notice a change after the first couple; tendon remodeling continues for weeks after the last session, so the full effect often shows up later.

Can I keep running?

Usually, yes, as symptoms allow. We suggest keeping the day of treatment and the day after easy, and avoiding big jumps in load while you heal. Coaching athletes get this built into their plan.

Should I take anti-inflammatories or ice afterward?

We generally suggest skipping over-the-counter anti-inflammatories and ice for a couple of days after each session, since the treatment works partly through your body's own healing response. Don't stop anything your doctor has prescribed.

Is this a replacement for seeing a doctor or physical therapist?

No. We don't diagnose injuries. If your pain is new, severe, getting worse, or you're unsure what it is, get it evaluated first - shockwave and EMTT work best on a known problem.

I'm a coaching athlete - how do I get my rate?
